The preceptor and agency details UNCW asks you to submit
For each NP preceptor, UNCW asks for a current CV, full name and credentials, a license number, a preferred email, and the agency's name, address, phone and county, plus two optional contact fields. These details feed the Clinical Placement Packet, one per preceptor, that the Graduate Clinical Coordinator reviews. Every field filled correctly on the first pass keeps your approval moving.

UNCW's list, item by item
The FNP and PMHNP preceptor pages on the Preceptor's Corner print the same list. Preceptors provide these "When prompted," which in practice means when you are building your placement packet for a specific course and semester.
What the preceptor provides

- Preferred email address, which UNCW uses for communications and evaluation links

The AGPCNP preceptor page leaves this list off, but the clinical handbook's packet applies to all three NP tracks, so AGPCNP students gather the same items. Treat the two optional fields as worth filling: they can decide how quickly site paperwork reaches the right desk.
The CV carries the most weight
UNCW explains why it wants the CV: it is "required for CCNE accreditation and preceptor approval." The reviewer uses it to confirm what the track pages ask for, above all at least one year as a provider and, ideally, experience at the site where you will train.
That is why UNCW is specific about two details. Experience dates show the year of practice is real and current, so month and year for each role beat a bare list of employers. And the site itself must appear under professional experience. A preceptor who joined a new practice in the spring may still be circulating a CV that ends at the old job, and that version fails the one test UNCW names.
Before you submit, find your planned clinical site by name on the preceptor's CV. If it is missing, ask for an updated copy first.
Name, credentials and license number
Full name and credentials tell the reviewer which set of rules applies. An NP or certified nurse midwife is checked for an RN license, Approval to Practice and board certification; a physician or PA for a North Carolina license and board certification; a PMHNP therapy preceptor for a full North Carolina license in their own field. UNCW asks for an "RN or medical license number" because that number is how a license is looked up. For an NP, that means the RN license number, not a DEA or national certification number. Match the name on the form to the name on the license exactly, including a hyphenated or changed surname, so the record is easy to find.
The email that carries evaluations
The preferred email looks like a formality, yet it is the channel UNCW uses for "communications and evaluation links." Preceptor evaluations run through Typhon, and the clinical handbook tells students to confirm that the preceptor has completed the Assessment of Clinical Performance before the semester ends in order to receive a grade.
Choose an address the preceptor reads every working day. A work address is fine, and so is a personal one, as long as links will not sit unread in a shared clinic inbox. Spell it back to the preceptor before you submit. Other messages follow over the semester too: the Clinical Placement Team documents each preceptor's completed rotation hours at the end of the term, a record NP preceptors can use toward the state's continuing education credit for precepting. Agency name, address, county and contacts
Use the agency's full name as the practice itself uses it. Large systems run clinics under several names, and the Clinical Placement Team uses this entry to check for an existing agreement, add the site to Typhon and send site-specific paperwork. The address should be the building where you will see patients, not a billing office or corporate headquarters.
County is its own field on UNCW's list, so fill it in even when the address makes it look obvious. The agency phone goes on the form as well; keep it to the main line the site answers.
The optional contact name, title and extra email are usually the practice manager or clinical education lead. If the site asks for an affiliation agreement, that person may be the one who routes it, and naming them up front can save days. The affiliation agreements guide explains that step.

Everything above becomes part of the Clinical Placement Packet, which UNCW describes as cover pages, a Partner Site Acknowledgement Form, a Student Placement Acknowledgement Form and the preceptor's CV, sent per preceptor. The Graduate Clinical Coordinator reviews it, and the form then routes to the preceptor through DocuSign for signature. If one site can cover only part of your hours, each added preceptor gets a separate packet, up to UNCW's limit on preceptors per course.
Timing matters as much as accuracy: UNCW asks for placement information at least 8 weeks before the semester, and its process chart says 90 days before the course. The 8-week deadline page walks through that conflict, and the placement process page covers each step end to end.

Questions UNCW students ask
Does my preceptor need to send a new CV if they precepted me last semester?
Plan on it. UNCW's placement packet is prepared per placement and per preceptor, and the preceptor's CV is part of each packet. Returning to a previous preceptor also needs the Graduate Clinical Coordinator's approval. An updated CV with current dates keeps that review simple.
Who fills in the placement packet, me or my preceptor?
You do. UNCW's handbook has students submit preceptor information through their Clinical Placement Packet form. The preceptor supplies the items on UNCW's list when prompted and later signs their pages, which reach them through DocuSign after the Graduate Clinical Coordinator approves the placement.
Is the agency contact person required?
UNCW marks the agency contact name, title and additional email as optional. They are still worth including, especially at a site that wants its own affiliation agreement or onboarding paperwork, because the Clinical Placement Team can reach the right person without a round of back-and-forth.
Which license number does a PA or physician preceptor give?
Their North Carolina medical license number. UNCW's list asks for an "RN or medical license number," and the FNP and AGPCNP pages require PA, MD and DO preceptors to hold a current North Carolina license with active board certification. NPs and nurse midwives give their RN license number.
Do nurse educator preceptors use the same list?
The list on this page comes from the DNP preceptor pages. MSN Nurse Educator and certificate practicums have their own approval by the Nurse Educator Programs Coordinator, with a signed agreement at least five weeks before the practicum. Your course materials name the details they need.
